2011-2012 Back to School Information

For the 2011-2012 school year, Friends School will be electronically collecting as much of our form data as possible. A limited number of forms will still need to be printed, signed, and returned to the School.

We will be sending email communications out to families as we move throughout the summer and below is an outline of the information that you will need.

• Physician Signed Health Forms

Parent Signed Back to School Forms

On July 18th, you will have received an email from the School with a link to the online parent Back to School form process. The online parent signed forms are due by August 12, 2011.

Student Schedules, Class Lists, Etc.

Information about student schedules and homerooms were made available in the Parent Portal on August 1, 2011.

Household Updates / Printed Directory Information

We ask that parents log into the Parent Portal and review and update the information on each item under the Household menu (Household, Parent 1, Parent 2 [if applicable], Emergency Contacts and Physician Information).

Please check your household and parent information by July 28, 2011 to ensure correct information for the printed 2011-2012 directory.

New families will receive Parent Portal login information after their child has been enrolled at Friends School.

Physician Signed Health Forms

Any form that requires a physician signature must be printed and returned to the school.  These forms are available by choosing the correct division in the Related Media section on the right.  The forms available on our website are listed below:

• Health Forms Packet (Required for all students)
• Friends School Medication Administration-Physicians Order
  (Special Form – if applicable)
• Food Allergy Action Plan (Special form – if applicable)

Please return these to the school by August 1, 2011. 

If you have questions regarding these requirements, please contact the School Nurse before June 6 and after August 1, 2011. You may contact Bonnie Hearn over the summer with questions.

 

Summer Reading

September 15, 2011: The Summer 2011 Reading is no longer posted. The Summer 2012 Reading will be posted in late-May.
